Pastors: The below list is to assist you in submitting your annual report to our district and global church in preparation for the 2026 District Assembly (June 25-26). Please Note: The NMI Local President reporting and NMI Convention Registration is included in the checklist. Please be sure to pass the link along to your NMI President.
It is of the utmost importance that all your reports are submitted on time. This year’s Deadline is May 15th (of course early submission is welcomed and encouraged!). We also need your church information which is included in the District Journal. It is critical that every credentialed minister (District Licensed, Ordained) reports annually. Any credentialed minister who does not report for two (2) consecutive years may be removed from the Roster of Ministers (Manual; paragraph 521, 532.8, 536.2, 538.9).
All reports are to be submitted online. The Annual Pastor’s Report (APR) worksheets are available, in English, Spanish, Portuguese, French and Korean. You can download as needed. The hardcopy forms are worksheets for preparing to complete them online.
All necessary reports are below, though all do not pertain to everyone. Using this list assists you in being sure you have submitted all of the reports that do pertain to you. Below the list you will find information and instructions concerning each report.

Annual Pastor’s Report (APR)
Your Pastoral Report is to be completed online. You should have already signed up for your APR login. If you changed your APR email address during the year and have not yet created a new Nazarene Account – email me immediately so you can enter your APR. If you have not yet confirmed that you can login, try it now! If you encounter problems or have questions please contact me and I will do my best to assist you.
The instructions and worksheet for your Annual Pastor’s Report (APR) are available for printing or downloading. These are available, in English, Spanish, Portuguese, French and Korean. NOTE: There have been significant chances to the APR. It is strongly recommended that you check the worksheet to be sure you have what you need to report. Once you have completed filling in the worksheet, go online to complete your report. Login to the APR; enter your email address and password. If you would like a copy of your APR contact the district secretary.
Annual Report of Evangelist
Any District Licensed or Ordained Registered or Commissioned Evangelist is to report to the District Assembly. If you have such a person in your church, please remind them of their need to report.
Annual Report of Ordained or Licensed Minister (Not Lead/Senior Pastor)
All credentialed (District Licensed or Ordained) ministers are required to report to the District Assembly every year. Any district licensed and ordained ministers who are not serving as lead pastor of a church are to report using this form.
Annual Report of Retired Minister
All retired ministers (Retired Assigned [RA] or Retired Unassigned [RU] are required to report to the District Assembly. This report is for them. Pastors, please be sure that any retired pastors in your congregation submit their report. In some cases, you may need to assist them in doing the online reporting.
District Assembly Certificate of Election/Assembly Registration
Elect delegates to the district assembly according to the Manual 201.2. Local churches and church-type missions in districts of 5,000 or more full church members shall be entitled to representation in the district assembly as follows: one lay delegate from each local church or church-type mission of 75 or fewer full church members, and one additional lay delegate for the next 50 full members, and for each successive additional 50 full members. (See chart below.) (24, 115.14-115.15, 201)
0 – 75 full members = 1 delegate
76 – 125 full members = 2 delegates
126 – 175 full members= 3 delegates
176 – 225 full members = 4 delegates
226 – 275 full members = 5 delegates
276 – 325 full members = 6 delegates
326 – 375 full members = 7 delegates
376 – 425 full members = 8 delegates
426 – 475 full members = 9 delegates
476 – 525 full members = 10 delegates
(For every 50 members above 525, 1 additional lay delegate.) Delegates must be elected at an annual meeting in accordance with Manual paragraph 113.12.
List only church elected delegates and alternates in the delegate section of the form.
The top portion of the form is for other members of the assembly (such as NDI President., NMI President, and NYI President). List only the names of people who will be attending the District Assembly.
Delegates (details above) must be registered in order to be a voting member of assembly. Please note: Delegates must be registered by May 15, 2026. Highlights and Goals Sheet for District Assembly Delegates Handbook
On this from you are to share a missional highlight from this past year in two to three sentences; share a need the district can join in prayer concern; a missional adventure you are planning for the upcoming year in two to three sentences and note if you are considering a New Start Church Plant. Brevity is the key word in this report. This report is included in the assembly handbook.
In Memory - Deceased Church Member Form
On the Deceased Church Member form, list the names of those members of your local church who have died since the previous year’s assembly. These names will be included in the memorial service at the district assembly. PLEASE verify spelling and confirm they have truly deceased!!!!! Names are copied electronically directly from the report.
Local Minister's License District Registration
It is critically important that if your local church has granted a local minister's license to a church member that we have their information. Please register any local licensed individual.
Overnight Accommodations
Hotels and Restaurants list for areas near Valley Chapel. All hotels are a 20–30-minute drive from Valley Chapel (the church's address is 14 Hunter Rd, Uxbridge, MA).
Breakfast and Dinner are on your own. Please see the list for restaurants near Valley Chapel (second page of the hotels link above). Lunch is provided onsite both Thursday and Friday at no cost.
Pastor’s One Page Assembly Report
You are to write a one page report (equivalent to a 8 ½ x 11 page, 1’ margins). This will be included in the report handbook at the assembly. Do not include any special formatting, graphics, or logos in this report. It is a highlight report. You can type it in a word processor and using the “ctrl-C” to copy and “ctrl-V” to paste, you can copy and paste the report into the online form.
